Beyond Trauma: Rebuilding the Self with EMDR

Clients who present with life-long relationship problems and serious self-esteem issues often have pervasive developmental deficits that manifest as attachment and/or character disorders. While EMDR is an efficient treatment, addressing the early life experiences that contribute to a client’s ongoing difficulties is more than just reprocessing the nodal events of childhood; it is about generating corrective experiences of self in relationship with others. Participants will learn how to use EMDR to facilitate the uncoupling of negative core beliefs and core affects from positive, more adaptive aspects of self that have been depressed, dissociated, or otherwise undeveloped. To this end, clinicians will learn how to actively utilize the client’s emergent experience of self within the context of EMDR processing using an expanded repertoire of cognitive interweave strategies. Materials will be presented through lecture and videotape of actual continuous clinical cases as well as a large group discussion format in which participants will be able to discuss their own cases with the presenter.

A.. Beyond Trauma Resolution: EMDR and the Growth of the Relational Self

This workshop explores family dynamics contributing to ego fragmentation and interlocking ego state conflict in individual and couples therapy. Participants will learn about the relationship between family dynamics and ego structure, reenactments from the family of origin, and the manifestations of ego state conflict in individual and conjoint therapy. Participants will learn a contextual model for EMDR-based assessment and treatment planning, the indications and contraindications for conjoint EMDR, a model of the Self that zeroes in on salient negative cognitions, and a progression of techniques to control and focus desensitization within the optimal zone of arousal.

B. Using EMDR to Overcome Roadblocks in Addiction Recovery: Clinical Applications Based on Phenomenological Inquiry

The presenter will apply the EMDR protocol to work with recovering addicts. EMDR can assist these clients in working through recovery roadblocks such as inability to express feelings in a healthy manner and improper perspectives on lifestyle change. The workshop will reference the findings of the presenter’s 2008-2009 doctorial dissertation, “EMDR in Addiction Continuing Care: A Phenomenological Study of Clients Treated in Early Recovery.” The presenter, who regularly uses EMDR with recovering addicts, will draw upon the literature and extrapolate the clinical relevance of her study’s findings in a way that is palatable to both scholars and practitioners.

C. EMDR: The Clinical File as a Clinical Tool

This workshop will present a simple and specific system for creating a clinical file, which then becomes not just a legal record, but a powerful clinical tool (templates of all forms will be provided, with case illustrations). The
AIP-informed conceptual framework for this system of trauma recovery planning will be outlined, with supporting research. Phases 3-8 are only going to be as good as Phases 1 and 2 - the clinician must be able to quickly access clinical information from the file at crucial moments during sessions. This system allows the clinician & client to know exactly where they are at all times. It will help clinicians to improve their clinical work by improving their record-keeping.

Le deuil et la dépression sont des expériences de vie qui présentent un défi pour les mécanismes d’adaptation et remettent en question les états d’équilibre déjà établis. Les liens qui deviennent de plus en plus évidents entre ces états expérientiels et la présence de mécanismes défectueux de traitement de l’information soulignent l’importance d’offrir des services psychothérapeutiques , telle l’approche EMDR qui tient compte de ces besoins et de ces processus spécifiques .Notre présentation veut souligner : 1) l’importance des protocoles et des principes de base de l’approche EMDR comme modèle d’intégration des processus de psychothérapie 2) les recherches qui explorent les mécanismes dysfonctionnels des réseaux de mémoire impliqués lors du travail du deuil et 3) les applications de protocoles spécifiques EMDR pour les diverses étapes du deuil. La partie expérientielle de cette présentation explorera certains des facteurs adaptatifs permettant le renouvellement de cet équilibre perdu et la consolidation de mécanismes reliés à la résilience.

A. Gaining Clarity …Driving your EMDR practice with the Adaptive Information Processing Model
Earl Grey Ph.D.

The EMDR clinician’s understanding of the tenets of the
AIP model and EMDR protocol is necessary for effective case conceptualization and treatment. This workshop is designed to assist the therapist in translating this knowledge into practical application of the AIP by adding an element of fun. Dr Grey is skilled at using analogies and narratives to make highly technical neuroscientific information user-friendly and understandable. His style is down-to-earth and engaging as he “plays” with the information to make it fun and interesting. You will leave the workshop with a stronger understanding of the AIP, 8-Phase Protocol, and the three-pronged approach; and having learned creative tools to apply to EMDR case conceptualization and planning.

Il peut être parfois un défi pour les thérapeutes EMDR à aider les clients à identifier les cibles de travail EMDR lorsque la personne se présente avec un problématique généralisée ou des enjeux « non-traumatiques » par exemple, certains associés à l’estime personnel, l’affirmation de soi, les habiletés relationnelles pour lesquelles des événements traumatiques « petits-t » où les liens ou expériences à cibler ne sont pas facilement identifiables. Cet atelier à pour objectif de permettre aux cliniciens à mieux aider leur clients à préciser d’avantage les enjeux et les cibles de travail potentielles avec l’EMDR; ainsi que de faciliter à ce que le client puisse, à partir de la problématique, de la cible et de l’image, mieux trouver les cognitions/croyances négatives activées. Dans ce processus, on vise aussi à accentuer l’importance de la phase 1 (l’histoire de la personne). Également, l’atelier se penchera sur l’importance de bien identifier/cerner la croyance négative sous-jacente au moment d’amorcer le travail avec les cibles identifiées, afin de maximiser les effets du retraitement et de favoriser la généralisation. 
À travers des présentations didactiques, des vignettes cliniques et des exercices de groupes, les participant(e)s pourront développer des stratégies pour mieux conceptualiser les plans de traitement EMDR avec ces types de problématiques. Également, les cliniciens seront amenés à réfléchir sur les thèmes des enjeux travaillés et leurs liens avec les cognitions négatives identifiées, sous les thèmes de responsabilité, sécurité et choix personnel.

C. Use of EMDR in the treatment of Obsessive Compulsive Disorder: a case study.
Elise St André

Obsessive compulsive disorder (OCD) is a chronic anxiety disorder with recurrent obsessions, like persistent thoughts, mental images, impulses, and compulsions such as repetitive behaviours or mental acts, that are performed after obsessions. This workshop will provide clinical information on the presenter's use of EMDR with a patient with OCD, as well as some examples provided by other clinicians who have worked with patients with OCD. In addition, general information on OCD (e.g., epidemiology, neurobiology) will be discussed. The presenter will highlight what is unique about the use of EMDR with OCD, difficulties she met in using EMDR with an OCD patient, and adaptations she used to improve treatment. She will describe how to use the standard 8 step approach (past, present, future), how to identify targets, managing OCD symptoms and barriers in his day-to-day life. The presenter will look at how to manage challenges that arise for the clinician with this population. Participants will have an opportunity to apply the strategies with a case example exercise.

D. Sex, Dreams and EMDR: Incorporating a focus on nightmares and dreams in therapy of sexual dysfunction
Robert K. Ferrie M.D.

There is an increasing body of neuro-scientific knowledge linking the mechanism of action of EMDR to that of REM sleep. This workshop will discuss how to treat nightmare disorders of REM sleep using EMDR by reviewing cases of traumatically caused sexual dysfunction. The workshop will explore how using EMDR to target dreams allows unique treatment effects to occur. A discussion of cases will be used to illustrate the application of the above findings of neuroscience to actual EMDR treatment of this particular post- traumatic disorder, which is so deeply imbedded in the body. Video portions of sessions will be provided.

A. The Case of Mistaken Identity: EMDR, Ego States and Attachment in the Treatment of Eating Disorders

In this workshop the presenter explores the presence of dissociation in clients with eating disorders, particularly anorexia nervosa. The approach described employs an EMDR phase model, with expanded evaluation and preparation phases. The extended preparation discussed includes a 4-step method of teaching emotional competence, an introduction to body awareness, and the use of ego state therapy with the disordered part(s). Processing typically requires attachment repair, as well as the ability to titrate released disturbance and re-stabilize after EMDR application to touchstone events. Video clips, case studies and case reviews will reinforce learning.

Le trouble obsessif compulsif (TOC) est un trouble anxieux généralement chronique se présentant avec des obsessions récurrentes tel des idées persistantes, des images mentales et des compulsions (suivant les obsessions) tel des actes physiques ou mentaux répétitifs.
Dans cet atelier, le médecin fournira des indications cliniques sur son utilisation de l’EMDR dans le traitement d’un patient souffrant de TOC, et donnera quelques exemples tirés de la pratique d’autres cliniciens, en sus d’une révision des notions de base utiles à la compréhension du TOC (théorie neurobiologiques, épidémiologie, etc.).
La présentatrice tentera de souligner ce qu’il y a d’unique dans l’utilisation de l’EMDR dans le traitement de l’OCD, les difficultés rencontrées et les solutions utilisées dans ce cas précis. Elle décrira l’utilisation de la méthode de traitement standard en 8 étapes dans un cas spécifique, et dans les modalités de temps (passé, présent, futur), Nous verrons l’identification des cibles de traitement, et la gestion des symptômes de TOC qui entravent le fonctionnement quotidien des patients affectés. Nous verrons aussi les défis proposés par cette population, en clinique. Les participants pourront utiliser certaines stratégies dans un exercice pratique.

C. Connecting to a Team of Resource Ego States to Prepare for EMDR Trauma Processing
Rochelle Sharpe Lohrasbe M.A.

It can be difficult to assess readiness for emotionally taxing EMDR trauma processing. The first half of this workshop will cover a protocol for helping clients connect with three resource ego states – a Nurturing Adult Self, a Protective Adult Self, and a Spiritual Core Self. Clients with sufficient ego strength for trauma work will readily connect to these resources. Clients with poor ego strength often encounter blocks when connecting to resource ego states. The second half of the workshop will cover an ego-strengthening intervention for clearing such blocks. This intervention can help prepare clients to tolerate trauma work.

A. Getting stuck: Navigating through the Protocol
Marshall Wilensky Ph.D.

The focus of this workshop will be the Basic Eight Phase Protocol and most specifically the Three Prongedness (Past, Present, Future) of EMDR. When the Basic Protocol is followed diligently many problems disappear. The presenter will draw on his experience as a supervisor and trainer of EMDR clinicians to demonstrate strategies to deal with mini-impasses in therapy. Questions are welcomed about : how to formulate a treatment plan and find the touchstone memories, how to get well-formed Negative and Positive Cognitions, things to do when reprocessing is stuck, what to do when clients have difficulty identifying emotions, evaluating VOC and SUDs and generating an image with appropriate detail? How does the clinician react when the client "loops" and doesn't seem to progress? Issues of secondary traumatization and vicarious traumatization of the therapist. These and other common questions will be addressed,with ample time for examples and a possible practicum. If EMDR is a church, then the presenter is close to a fundamentalist. We all drift. Let's get back to Basics.

B. Integrating EMDR into a long term psychodynamic treatment
Gina Colelli M.S.W.

This workshop will cover specific strategies in implementing EMDR in the creation of a psychodynamic treatment plan. The workshop will cover target selection, pacing of treatment, using EMDR with transferences and defenses, and processing characterological symptoms to a state of resolution. Participants should be well versed in a psychodynamic method and the use of EMDR. The workshop will be specific to how to integrate the methods for successful goal oriented treatment.

C. Helping Children Shrink the Worry Monster Utilizing EMDR
Jan Yordy M.Ed., M.S.W.

An increasing number of children are suffering from anxieties, stress and even obsessive and compulsive behaviors. Different EMDR approaches are helpful for children in facilitating the release of anxieties and altering primitive brain patterns. This workshop will briefly describe some of the causes of children’s anxieties, the brain/body connection to anxiety and some simple calming techniques for releasing stress. In addition, an in-depth introduction to three EMDR child-friendly techniques for working with anxiety and trauma will be described. Case examples for each technique will be utilized to enhance the understanding of the three therapy techniques.

A. Mindfulness and EMDR

This session explores the synergy of Mindfulness and EMDR, specifically in relation to strengthening client affect tolerance, body awareness, observer capacity and self-acceptance. These skills are particularly useful in the Preparation phase of EMDR in order to build a strong foundation for the Desensitization phase of the Trauma Protocol. We will look at Mindfulness techniques, combined with Resource Development and Installation (RDI), that are useful for both rigid/conceptualizing clients and chaotic/overemotional clients. Research linking Mindfulness with neural plasticity and neural integration will be highlighted. The session includes lecture, case review and short experiential Mindfulness exercises.

B. EMDR: Containment and Closure

This workshop will focus on the importance of containment in EMDR and its role in helping clients with affect regulation. It will look at containment in EMDR’s Phases 2 (Preparation) and 7 (Closure). An AIP-informed rational for containment will be offered, with supporting research. By learning an array of strategies for containing negative affect, participants will increase their ability to properly close incomplete EMDR sessions. Experiential exercises will enable participants to practice new methods before using them with clients.

A. Musical Interweaves in EMDR Treatment
Jamie Marich Ph.D.

Spatial intelligence is emphasized in traditional EMDR protocols and instructional methods when focus is placed on imagery. Bodily-kinesthetic intelligence is honoured when body cues are addressed. However, individuals with primary musical or sonic intelligences are not optimally attended to with orthodox application of well-accepted EMDR strategies. Without making any changes to the stages of the Shapiro protocol, musical interventions can be implemented into EMDR treatment to amplify the holistic design of EMDR and to optimally serve those who struggle with spatial intelligence. In this workshop, participants will learn how to utilize music in resource development and EMDR preparation, to apply music as an appropriate “cognitive interweave” when processing is not flowing optimally, and to consider the impact that music can have on EMDR treatment. The presenter will implement a combination of lecture, participation activities, and case discussion to achieve learning objectives.

B. Baby Ache: Applications for EMDR in Infertility, Miscarriage, and Perinatal Loss

Infertility, miscarriage, and perinatal loss can be among life’s most deeply painful experiences. Losses associated with these life events can be traumatic. For individuals and couples accessing fertility treatments, pre-existing trauma and phobias can complicate their efforts to achieve a successful pregnancy. This presentation outlines common emotional responses to infertility and reproductive loss. It reviews the role of EMDR in treatment of trauma and phobias and applies this knowledge to clients experiencing difficulty conceiving and/or maintaining a pregnancy. Case studies illustrate how EMDR has been used with this client group.
